Until Now — My Medical Family provides medical guidance, care coordination, and clinical oversight at home, so you can continue living at home while dealing with chronic or serious medical conditions. We become the medical family member you wish you had, the one with the professional expertise you can trust.
Who We Are
My Medical Family is a private healthcare company created by experienced clinicians to serve aging retirees who do not have family close by to help manage their growing medical needs.

The experienced founders have led large healthcare organizations but now have chosen to build a new model of care focused exclusively on the individual — not the institution.

We have also personally cared for aging parents — and understand the burden families face trying to preserve quality of life and dignity at home rather than in nursing homes or institutional care.
What We Do
A My Medical Family clinician begins by visiting you in your home to perform a detailed medical history and physical examination, review your electronic medical records, and discuss your personal health and independence goals.

We then develop a clinical assessment and personalized plan for how we can help promote a healthy, safe, and active life at home.
​
Your nurse care manager and doctor will visit regularly to monitor your progress, manage your care plan, and coordinate services recommended by your physicians and specialists.

Your doctor and nurse care manager are highly accessible through our telehealth portal.
