How We Charge for Our Services
Office-Based Primary Care Practice
My Medical Family, P.A., participates with select commercial insurance plans and accepts Medicare Part B for covered physician services. We also provide services to patients insured through Community Health Partners (CHP). Self-pay arrangements are also available.
Participation in an insurance network varies by specific plan product and employer group. Inclusion in a carrier’s network does not guarantee participation in all plans offered by that carrier. Patients are responsible for verifying network status and benefit coverage with their insurance carrier prior to scheduling services. Coverage determinations, benefit levels, prior authorization requirements, and reimbursement decisions are made solely by your insurance provider.
Out-of-Network Insurance
If we are out of network with your insurance plan, payment is due at the time of service. We will provide a detailed receipt that you can submit to your insurance company for possible reimbursement. Reimbursement depends on your individual plan. We will provide a Good Faith Estimate of expected charges before your visit, as required by federal law.
Home-Based Practice (All services except In-Office Primary Care)
Most in-home services offered by My Medical Family are not covered by Medicare or commercial insurance plans.
For these services, patients are charged:

A one-time initiation fee, which includes a comprehensive assessment and individualized care plan; and

A recurring monthly subscription fee, based on anticipated level of service needed.
Special patient administrative assistance (e.g., management of complex third-party healthcare bills) and support services outside of physician/nurse care manager time are billed separately.
Important Medicare Disclosure
Enrollment in our subscription services is voluntary and is not a condition of receiving Medicare-covered care from My Medical Family, P.A. Patients may choose to receive Medicare-covered services without enrolling in subscription services through our office-based practice.
We do not bill Medicare for services included in the subscription that are not covered by Medicare.
