We offer two distinct but complementary models of care—traditional in-office primary care and concierge in-home care—so individuals and families can choose the approach that best fits their needs.

In-Office Primary Care
​Insurance-Based, Fee-for-Service Medical Care
Our in-office practice provides comprehensive primary care for adults who prefer to receive their medical care in an office setting and want an ongoing relationship with a trusted physician.
This model is appropriate for individuals seeking routine primary care, preventive services, or management of stable chronic conditions.
What this includes:
• Preventive care and annual visits
• Diagnosis and treatment of acute illnesses
• Management of common chronic conditions
• Referrals to specialists when needed
• Coordination of care within the broader healthcare system
How it works:
• Insurance-based, fee-for-service model
• Accepts Medicare, certain insurance plans or self-pay
• Designed for adults who want accessible, high-quality primary care in an office setting
​Concierge In-Home Care
Subscription-Based Medical Care for Older Adults
Our concierge in-home care is designed for older adults who want personalized, ongoing medical care delivered in the comfort of their home, including those with chronic, complex, or serious health conditions.
This model allows our physicians and clinical team to spend the time needed to understand the full picture of your health and daily life—beyond what is possible in an office visit. Care is proactive, continuous, and customized to your needs and goals.
What this includes:
• Physician and nurse visits in your home
• Comprehensive medical assessments and ongoing management
• Complex care management
• Post-hospital or post-surgical follow-up at home
• Coordination with specialists, hospitals, and home health services
• Support and guidance for family members and caregivers
How it works:
• Subscription-based service. Fees based on anticipated level of service needed.
• Accepts ACH, credit card or check payment in automated or monthly payments.
• Designed to reduce fragmented care, confusion, and unnecessary hospital visits
• Ideal for individuals who prefer or need care at home and require a higher level of medical involvement
This service is especially well-suited for those who value continuity, accessibility, and peace of mind, and who want a physician closely involved in their care.

​​How We Charge For Our Services
​
Office-Based Practice
My Medical Family, P.A. participates with select commercial insurance plans and accepts Medicare Part B for covered physician services. We also provide services to patients insured through Community Health Partners (CHP). Self-pay arrangements are also available.

Participation in an insurance network varies by specific plan product and employer group. Inclusion in a carrier’s network does not guarantee participation in all plans offered by that carrier. Patients are responsible for verifying network status and benefit coverage with their insurance carrier prior to scheduling services. Coverage determinations, benefit levels, prior authorization requirements, and reimbursement decisions are made solely by your insurance provider.

​Out-of-Network Insurance
If we are out of network with your insurance plan, payment is due at the time of service. We will provide a detailed receipt that you can submit to your insurance company for possible reimbursement. Reimbursement depends on your individual plan. We will provide a Good Faith Estimate of expected charges before your visit, as required by federal law.

Home-Based Practice
Most in-home services offered by My Medical Family are not covered by Medicare or commercial insurance plans.
For these services, patients are charged:

A one-time initiation fee, which includes a comprehensive assessment and individualized care plan; and

A recurring monthly subscription fee, based on anticipated level of service needed.​

Special patient administrative assistance (e.g., management of complex third-party healthcare bills) and support services outside of physician/nurse care manager time are billed separately.

Important Medicare Disclosure
Enrollment in our subscription services is voluntary and is not a condition of receiving Medicare-covered care from My Medical Family, P.A. Patients may choose to receive Medicare-covered services without enrolling in subscription services through our office-based practice.

We do not bill Medicare for services included in the subscription that are not covered by Medicare.
