All patients are charged an initiation fee and a monthly subscription fee.

 

For Medicare patients, we accept Medicare physician fee rates for covered services.

 

For non-Medicare patients, the initiation fee includes the initial assessment and care plan.  The monthly subscription covers physician, nurse care management, and advisory services.

Special patient administrative assistance (e.g., management of third-party healthcare bills) and support services (e.g., transportation to doctors’ appointments) are billed at hourly rates.

The monthly subscription fee is charged automatically based on the expected level of service.  There are three categories of subscription services: Low, Medium, and High.  The level of service will be reviewed every quarter and adjusted as appropriate.
