Concierge Subscription ​​

​

All patients are charged an initiation fee and a monthly subscription fee. The initiation fee includes the initial assessment and care plan.  The monthly subscription fee is based on the expected level of services required and covers physician, nurse care management, and advisory services.

For Medicare patients, we accept Medicare physician fee rates for covered services and will discount the initiation fee and monthly subscription. 

​ Special patient administrative assistance (e.g., management of complex third-party healthcare bills) and support services outside of physician/nurse care manager time are billed separately.
